what's the dimension of the grab rail?  and what's it made of?
if it's at least 1.5" across at the point where you're drilling, it
would probably be okay.  another thing to consider:  securing a long
metal bolt through your grab rail sounds like you'll be tightening
nuts against the grab rail -- this will almost certainly mar the
appearance. 

if you're worried about it, have you considered putting a top
rack on in place of the grab rail?  you could then either clamp to or
drill through the rack, or attach the shaft some other way to it.
